- про российскую медицину по страховке (не о смерти пациента на операционном столе, потому что отчего-то оказался закрыт кабинет переливания крови, и не смерти младенца - в брошеной на лоток вырезанной матке оказался живый плод, но врачи не предвидели такую возможность - застрявшие в памяти давние рассказы бывшей московской, а ныне бернской медсестры - брак со швейцарцем по большой любви - не об этом, а об обыденном и своем, как оно там сейчас у моего отца)
- про американскую медицину - не о 40 миллионах без страховки - я знаю немного о том, как это получается, и про точку зрения юзера bbb мне есть что сказать, но сейчас не об этом, сейчас о pro-bono операциях, про это надо говорить тоже
- не первый раз - про детей с проблемами и про спец. программы для детей с проблемами (тут и Нью-Йоркер пригодится, неожиданно).

I can't get the 40 mil without health insurance. The first thing that my friend upon her arrival to the US was obtain freecare. Please note that freecare is not medicare (my friend was not eligible for that).

It seems to me that very poor people can get insurance. I suppose that there are people who are not below poverty line but cannot make the monthly insurance payments either...

Correct (with some exeptions). It's not those below the povery line who make these numbers. I shall post re subj later.

It's those who go freelance, or are starting their own company/business, or work for small business that does not have luxury to provide its employees with health benefits. I just recently read in TIME that very modest private coverage (most like catastrophic one) costs over $200 for one person and about $800 for a family of 4 (per month). Make your own conclusion.
